Coronavirus Notice to Customers / Clients. COVID 19 Safety Protocol for the re-opening of shop / restaurant with liability disclaimer / waiver / declaration from customers that they assume the COVID-19 risks and hold the shop / restaurant harmless from liabilities.

COVID-19 Safety Protocol and Liability Waiver / Disclaimer / Declaration

(the “Company”) and the Customer will adhere to the following Novus Coronavirus (“COVID-19”) safety protocol:

1. Health screening – the Customer will be subject to health screening by the Company including taking temperature and other symptoms assessment prior to entry into the  of the Company.

2. Face covering – the Customer will wear a mask or face covering when at the  except when consuming the food and beverages of the Company.

3. Face covering – the Customer will wear a mask or face covering when at the  of the Company.

4. Social Distancing – the Customer will practice social distancing and stay at least 6 feet from other people (except for members of the Customer's group) whenever possible.

5. Sanitisation – the Company will disinfect and clean its , with more frequent cleaning of high touch surfaces.

I (the “Customer”) hereby declare and agree that:

1. No Symptoms - I do not have and have not had in the past 14 days any of the following symptoms fever, malaise, dry cough, shortness of breath or other flu-like symptoms.

2. No Travel - I have not been out of in the past 14 days.

3. No Contact - I have not been in close contact with any person who has travelled outside of or with any person who is a confirmed or preliminary positive case of COVID-19 infection in the past 14 days.

4. Assumption of risk — I am aware of the inherent risk of COVID-19 at the Company’s premises and is willing to take the risk.

5. Covenant not to sue — I promise not to sue the Company should I be infected with COVID-19 at the Company’s .

6. Waiver of liability — I waive the right to hold the Company liable for any COVID-19 infection.

7. Indemnification — I will indemnify the Company for any claims against the Company relating to myself, my guests and my family.

8. Responsibility for Others - I understand that I am also responsible for the declaration of my guests and family members.

9. Confirmation - I confirm that the above information is accurate to my best knowledge, and agree that such information will be processed and used in accordance with the Company’s Privacy Policy Statement.
